Please note that though we are happy to complete forms on your behalf, there is a charge of $50 for most forms that are required by Human Resources, Government Agencies, Attorneys, Schools and Life/Disability Insurers. These forms take time to complete based upon your individual needs and require Dr. Saperstein’s expertise, signature and approval. To meet our patients’ needs, we have a contractor who takes care of this for us.
Our fees are as follows:
- FMLA Paperwork $50
- Disability Paperwork (ST or LT) $50
- Accommodation Paperwork $50
- Service Dog/Emotional Support Animal $25
- Jury Excuse $25
